In regards to the link to D.T. Max's New Yorker piece on Wallace (March 2009), a reader asks Max if DFW ever engaged in psychotherapy. Max replies: "I don’t know whether he tried psychoanalysis itself. I can’t imagine he wasn’t curious about it." Wallace's "Good Old Neon" suggests that he did, indeed, have at least one experience with a psychotherapist, with DFW writing again from a base of observational reportage, not pure invention.

With respect to #23, I believe that Max is incorrect. I'm not sure how you define "major" interview, but Wallace did an exclusive interview with the Wall Street Journal in May 2008 (just four months before he died) on the release of "McCain's Promise". I'm fairly certain this is his last interview.

Regarding #19: Possibly of interest: That student’s quote seems to have been taken verbatim from the DFW voicemail YouTube video:

Love this. Also loved that you thought that his students might forget who they were. I'm still laughing about that.

One thing: FN1: the book is The Broom of The System, not Broom of A System.

+Two of Wallace’s books not shouted out explicitly in the article:

Another one is Signifying Rappers (with Mark Costello).
